On point #3, that is the exact language that governs our state's reviews of utility rate increases and home ins increases and they NEVER reject them. Never. So I don't hold out hope of that process being very useful.

1) I haven't gotten to the effective dates for the individual mandate, which is really your question...I'll tell you as soon as I find them.
2)The 10K is a copayment limit, not a premium. I'm still picking my way through the subsidies to see how they fall.
3)Here's the exact sunshine language, which is a bit...bony right now.
The Secretary of Health and Human Services, in con- junction with States, shall establish a process for the annual review of increases in premiums for health insurance coverage. Such process shall require health insurance issuers to submit a justification for any premium increases prior to implementation of the increase.

1. Is immediate coverage "available" OR is it REQUIRED?
2. WHat if you can't afford the $10K family premium you mentioned (and they would need to reveal the amount the deductible for this categiory)
3.Premium increases must be reviewed? By WHOM? And how long before the Co's are just greasing the pckets of the politicians (I assume) that will be approving these increases so they sail through (like happens in my state with utility co rate increases and home insurance rate increases)?
